Tower inserts coming out

Has anyone experienced the thread inserts coming loose or being pulled out?
The thread inserts are in the tower mounting brackets, where the tower bolts to the brackets.
What are some solutions?

Maybe you could post a picture? I have a monstertower so I can't compare. Generically, threads can usually be repaired with helical (helicoil) inserts as well something generically known as a Keensert. Both are available at McMaster Carr - the bible of all mechanical engineers. Go to www.mcmaster.com and search on thread +insert. I believe the catalog page is 3062.
